Your health is your most valuable asset! For no more than what you most likely pay for your monthly cable TV access you can have access to quality health care providers. Immediately, upon enrolling on-line, you are able to access care from your chosen primary care provider. Your $25 per month subscription fee is deducted automatically from either your checking or savings account on the 5th of each month (or the closest banking day) for the previous month’s services. You pay the $5 per visit fee at the provider’s office - no more paper bills!

Please keep in mind that once enrolled, a Columbia Care contract is for one year. This means you are contractually obligated to continue paying your subscription fee for one year from your enrollment date. You will also be responsible for any services not covered by your monthly Columbia Care subscription fee.
